Just wondering if you can paint the ugly rusty wheel behind the rim?

That is the brake drum yes you can paint it
Paint mine once a year to freshen it up

Your local auto parts store should sell a caliper painting kit, this will work just fine for the drums too. Just be sure to clean all of the surface rust off first.

Any paint would work, it's just prep to make the part pretty before you spray. It's your brake drum...NOT another wheel behind your wheel. Just pull your wheel off, take a wire brush to the drum(that's still installed on the truck) to clean it, wipe or blow the rust dust away, then spray it. Don't worry about getting paint on the wheel studs(the threaded things), when you run the nut on it it'll clean the threads.
What you use is really up to you. I've used colored stuff before, I've used 'hammertone' stuff before, but i'm pretty stuck on a paint called John Deere Blitz Black.

I use Tremclad rust paint ,its cheap and if you put a few coats on it will last a long time .

I would use high temp, brakes get HOT
