As C clips wear over time, that tolerance increases. On some high mileage vehicles you could be seeing an eighth of an inch in play, but we don't know how many miles OP has on his truck

If the wheel pulls straight and evenly out a small fraction of an inch, that is normal. If the wheel moves out unevenly (i.e. if you rocked the top/bottom of the wheel) that is not normal.

If your muffler is only a single outlet, then you can't just cut a hole in it and run a dual outlet. You would need a single in/dual out muffler to start with.
