Well the belt is in fact toast. Next question to put on new tensioner or not. The bearing is still smooth I can spin it and it spins and spins. My thought is to trust it over a new Chinese made one....

I was no able to see the triangle but I did notice the belt is extremely loose. I am 99% sure the belt/tensioner is the problem. I am in the process of tearing into it. If it isn't the problem it is...

Thanks. The fuel filter was changed not to long ago. The truck is daily driver I just rarely fill the tank. Bad fuel is a possibility but I use the same station all the time.
