Hesitation through gears

Ok I have a 2003 reg cab short bed 5 speed duratec...99,000 miles...origional plugs wires and probably fuel filter...
going through the gears....sometimes it runs fine..and then sometimes around 2200-2500 rps it gives a little jerk like the motors lugging...Mainly does this under load..and only at moderate to regular driving ...
If your gettin on it.....The truck doesnt miss a beat...
What do you guys think?....Just needs a tune up ?
Its not throwing any codes
It idles fine
Has a fresh air filter....and ive cleaned the whole upper intake and sensors!

Stock service interval on plugs is 100k..........but most are toast and/or crap by 75k. You needs a tune up son.

Fuel filter intervals is 30k.

I'd say your truck has some catching up to do...
