Pull straight up on the old one. Drill into the new handle a hole that is slightly smaller in diameter. Then, tap the new one on the end of the shift lever to leave imprints of the "teeth". File those imprints to remove excess material and allow it to fit snugly onto the shaft end. Tap it home with a soft mallet and it is done.
I coated mine with a few coats of poly varnish to make it smooth and a little glossy in color.

I had the piece of elk antler given to me by a friend who has an elk and deer ranch. My intention was to make grips for revolvers out of it. When I had this part left over, and I saw the natural curve and how it fit in my right hand, I decided that was the section that would make the most comfortable grip when shifting. My hand now rests on the handle a lot of the time.
So I recommend that you try the antler piece you want to work with and make sure that you orient it in the way it is most comfortable. It's not enough that it looks good, it has to feel good in your hand or you will not want to use it.
