Did the rotors have new pads or used pads?
Used pads have a wear pattern that can warp a rotor pre-maturely.
Or continous hard stops or excess heat was placed on the rotors and warped them.

Both times at dealer with oem parts & new pads not driven hard or sudden stops

It could happen that seized pad can cause unnecessary friction to warp a pad. The dealer could have changed the pad or not, and say they did, but its hard to prove them wrong.... I dont know how it works over there but here in canada you can ask the garage for your old parts that they replaced that way you can see if they did change the pads and discs cause you will have them in your hands. Its also good so you can see for youself how your parts have perhaps failed. Asks for your old parts, by law in canada they have to give them to you.

Another thing that will cause them to warp is if you are driving and get them hot, then drive through a puddle where they cool down quickly.

how warped are they? too warped to be turned? one thing that can warp rotors is say you brake real hard for a stop light and keep your foot on the brakes. the pad takes all the heat away from that section of rotor and can warp it.
but from the sound of it yours must be pretty bad

I have seen that on other vehicles but it does happen. What I have always done is just put aftermarkets on, they last longer, hardly warp and they are good to go

i remember the exact point where i warped my rotors, i was in the city and there was a green light, then it went from amber to red in like 3 seconds and i didnt want a redlight ticket. thats the only problem ive ever had with warped rotors. x2 on the aftermarket pads, get some raybestos or wagner pads and rotors

Hard to believe your having several batches of bad brakes. I'd stop going to the dealer. My 02' that I leased brand new had an issue like yours that was covered because of a "factory defect". Driving only highway miles, I needed work under 15,000 miles, and I was surprised the dealer took care of me. But remember dealers get reimbursed by Ford for warranty work. After that. I never had an issue with that vehicle.

However, my 04' lease was a 4*4 and doing a search on here, you will find lots of owners upset with the quality of the back brakes. I needed a full brake job at around 30,000 that was out-of-pocket because of a quality control issue. My brakes were like sandpaper when they were removed.

My 08' that I own had to have the front brakes worked on after 30,000+ highway miles, and I went to a National chain. Premium NAPA semi-metallic pads were put on, and they needed to be adjusted twice due to squelling. They have since quieted, but I won't use them again.

Hopefully, you get things straightened out.
