If you have been changing the oil regularly, I doubt it is engine sludge, even if you haven't been changing the oil.
Have you run any injector cleaner through it?
How about changing the air filter, they get dirty and don't allow enough air into the engine.
Clean the MAF, they make a special cleaner for it, about $6 is a few year supply, unless you use it for other stuff. Don't use brake or carburetor cleaner, many say it ruins the MAF, I don't know that to be a fact.
I am sure others will come along with other things that could be causing your problem, these are just where I would start.
Let us know if it helps.

Do you ever get your truck on the freeway at 80+ MPH?
It actually does more good than harm to do it ever so often, so long as you don't get a ticket. Puttering around town will mess an engine up faster than you can believe.
If you can't find a place to go fast, go slow, but in a lower gear and wind the engine up. Kinda watch the exhaust and see whether you get a grayish cloud out of it.
Anyway, follow the link, read the article, and give it a shot. You might also run some Lucas or other quality injector cleaner through your system ever so often, I do it about every 5th tank of gas. If you go with Lucas, just buy a quart when you see it on sale, it keeps well. and will last about 20000 miles.

Yes.
Some will argue it is a waste of money, it isn't.
