I don't know anything about diesels or DEF, but I was recently reading about some of the VW "diesel gate" vehicles having a similar problem when they reached zero on some sort of AdBlue gauge. I believe they were saying that the dealer or someone with a programmer had to reset some variable in the ECU because it had a "I'm not going to start without any AdBlue" sensor that wouldn't reset by just re-filling the reservoir. I have no clue if that could be your problem, but it might be a place to start?
