Default Scratching noise under load

I've got a 99 3.0l 2wd automatic, 188k miles. I'm getting a noise like scratching or scraping under load, like accelerating up an on ramp to a highway or climbing a steep hill. I am fretting transmission trouble, anyone experienced this symptom? I have no warning lights. Otherwise seems to be driving and shifting fine.

Air leak? Does it run fine otherwise?

Air leak, I had not considered that. The truck otherwise is running reasonably well. About a year ago, I had a mechanic reseal the engine. He removed the engine and did the oil pump, oil pan, timing cover, and some engine seals. He did not do the exhaust manifold gaskets, which we really should have done. I coincidentally replaced the stock exhaust with a walker cat back system. In my opinion, I am getting a lot of piston noise that I had not noticed before. I am wondering if there are some exhaust leaks now contributing to the noise. Maybe I just don't care for the walker exhaust. The scratching noise is new, and happens if I really push the engine to accelerate on a hill, like merging onto a raised highway.

Leaking Exhaust headers sound like bad lifters, more of a ticking sound

Are you sure it's not a heat shield rattle?

This is a pretty good guess and makes sense to me.

Pistons don't make a scratching sound, more of a slapping/knocking sound when they go bad and compression would be dropping.

Actually, spent tonight working on my daughter's subaru, which has a caliper sticking. I need to spend a few minutes under the truck and see if I can find anything loose or rubbing somewhere. I don't think the tires are spinning, but that was one thing that occurred to me.
