I would go with 4.88, that would put you at 2,350 rpm in 5th gear at 60 mph. 4.56 would work but I think 4.88 would be better.
I'm not sure, but I'd assume you'd need to weld on new spring perches, maybe shock mounts, maybe an adapter u joint, maybe a driveshaft depending on how everything falls into place but I doubt you'll need a driveshaft. Hopefully someone who knows for sure will chime in.
