Default Potential problem with one of the Catylitic Converters

Hey guys, I've got an issue with part of my exhaust (i hope it is at least). For starters I have the 4.0 v6 with 121k on it. Now, there is a cat that sits right up under the engine about it in the middle, and when the truck is idling you can hear what sounds like a grinding noise from the exhaust. I know that this is not the timing chain guide because that sound was present well before this one ever showed up (and it only makes that noise on cold days). Now when I accelerate or put the engine under any kind of load this sound persists and gets slightly worse until I let off. Is this the sound of a clogged cat, or one that is slowly disintegrating? Or something else altogether? And yes I did use the search function, just didn't find what I was looking for...Thanks!
