Your door code should tell you what originally came on the truck. A trans code of D would be the Mitsubushi FM146. A M would be the M5OD-R1. It lists the M5 as the 2WD trans, but someone could of swapped a 4x4 version in, thought doubtful
Though looking at it is much easier. The M5OD-R1 will have a non-removable bellhousing. The Mitsu however does. Mitsu shifter plate is merely a cover that the shifter bolts to, whereas the M5, the shifter and shift rails all come out as one unit, off the top of the transmission via a large (6x10) aluminum cover. The shifter on the M5 is also on the "main body" of the trans case, whereas all mitsu shifters are located on what you would consider the "tail housing"
If you are 100% sure your truck is original, then this should get you what you need. If you suspect there has been a swap in its life, post a picture. They are all easily identifiable from each other.
To add to the mix, if it was a very early 88 4x4 its possible it came with a left over FM145. This will have a completly different shifter than the 46. The 45 will have a shifter that attaches to the "rear" of the shifter mount plate, while the 46 will have one that attaches to the "front" of the mount plate/cover.
