I know you have said that you've cleaned the IAC and checked it while the engine running but try unplugging it while the engine is off/cold and would not normally crank. Then try starting the engine normally while the IAC is unplugged without pressing the gas pedal.
I unplugged the IAC before I started it today - totally cold motor. It cranked and tried to start, but wouldn't start without pressing the gas pedal. After I plugged it back in, it cranked a bit but wouldn't start until I had the pedal floored. After that one time starting with the pedal floored after I plugged in the IAC, it starts normally now. It must have gotten flooded with the IAC unplugged. Maybe the IAC is working but not 100%???
