It has a new air filter. where is the fuel filter? It may need new wires as well because it starts hard but only once every few days.. Not sure why? it has nothing to do with weather because it does it at random times...

Tune up, Clean the throttle body and MAF, New plugs and wires.
Anything you can think of. New filters and fluids as well

That's ridiculous mileage... look at all the suggested and don't forget the pcv and I'd really look hard at that new exhaust. Plugged/malfunctioning cat or muffler? dent pipe or crushed closed blocking off the exhaust?

Question Re: terrible gas mileage need help!!!!

ok im going to check the maf and throttle body tomorrow. definetly going to go over the exhuast again.. Ive read on here about the wrong size plugs being used and the ones in it are almost new. maybe they are too short as others have said.. I plan on getting a new fuel filter soon to put in.. hope it changes something.

now getting around 6mpg highway.. and its lugging really hard around 55.. cant even get it to stay at 65mph..

I would see about checking compression also. Not sure if low comp. would equate to low fuel mileage.... but might explain the hard starting....?

I checked the muffler system and everything is good on that.... Checked the maf.. its a little dirty but nothing major.. the fuel filter is rotted to a t* and dripping..Its original lol. but they replaced the rubber tubes? who does that..anyway I bought a new one and im going to put it on tomorrow... found a serious problem while I was under there tho.. aparently they patched my block and the patch decided to fall out today and empty my coolant all over the place..and all over me.. I put some preston anti-leak in it.. its a small hole (about the size of a needle hole)in the front of the block but when its cooling down it poors out.. I dont have the money for a new engine so it looks like Im going to have to figure out a better way to patch it.

I know j/b weld does NOT work on blocks.. atleast it hasnt for me. (unless maybe they are brand new and spotless) I might just have to use some sort of liquid steal and see what happens.

plugs/wires are the least of my worries now. but they will still be replaced when I have the money to do it.

I was going to suggest a leak...looks like you found that (fuel filter and lines) plus one maybe worse..the block!!! If the hole really is that small (maybe your wrong about that..maybe below the surface there are cracks) perhaps a local machine shop, welder or mechanic can think of a fix. Being fuel injected even when off (the motor) pressure would be kept in the lines..continual leaking. Get that fixed and it may solve drive-ability issues. But the block leaking....while you can't afford it now...is the truck in good enough shape for you to put a re-manufactured motor in eventually? If not you may not want to spend much on it or keep it long.

good find on the fuel filter.. but a leaking block is no good. but its leaking coolant and not fuel so i dont think that could cause the terrible gas mileage. all that would really cause is a hot engine. since the cooling pockets are seperate from the rest of the engine, but since the spark plugs aren't rusted thats a good sign that the coolant isnt leaking into the intake or combustion chamber. like it was stated before low compression could be a good cause... is there any sign of white smoke or black smoke at all from the tailpipe? cause white is either a engine running lean or burning oil from worn out rings. black is a rich mixure so that could be a cause. the fuel injectors could be shot since the truck is older. or if you really want to dig deep the voltage across the fuel injectors could higher than normal which would open up the injectors alot more than normal.
