Default Cruddy engine behavior lately on 2002 4.0l 230k miles

So lately, the truck has been having issues starting and I have to hold the starter for 4+ seconds while slowly putting in and taking out throttle. It does seem to get worse when its colder out but happens regardless of the weather. Once it starts, it idles very badly, it reads less than 500 rpms and gets super lopey and theres a very strong exhaust smell, but doesn't die. Usually, revving it up hard 3 or 4 times will cause the revs to hang at about 2k for a few beats on the way back down. However last night, i hadn't driven it since the night before (at least 24hrs) and it had snowed and frozen during the day, and it started in the way i described, ran okay for a few 30 seconds, dropped to 100 rpms for a few beats and died. I started it again and it instantly died, this happened 2 more times until I got a good start and revved it out. It seemed to idle low but consistently after that. However this time when i I revved it, it would just touch a stall after I let off and come back up to 500rpms or so but wasn't dying. Drove and idled okay after that 700rpmish. Started okay a few hours later with the exception of a gun shot backfire out the intake which ive come to be used to after dealing with it for a few months

Some history
-230k
-New clutch,
-New plugs and wires
-New elbow of of manifold after fining huge vacuum leak
-Engine code for bank 2 running lean that comes and goes for a week or so at a time after having it for 6 months.
-Started very well and easily ever since ive had it until the last couple months
-sometimes it snows, the engine will shit the bed on a few rpms and cause a big buck but keep driving fine before and after causing no issues other than now brown tighty whiteys.
-no custom engine work
-recently cleaned and reinstalled but original iac and throttle.
-very small vacuum leaks are sealed not replaced
-odd rattle noise behind the dash that gets worse when slightly turning the wheel and the volume is speed depending.
-all interior lights left of center of the steering wheel went out within the last week (headlight and dash brighness dial, tach, water temp, fuel; warning lights still work fine)
-gas leaks when I fill up all the way.
-gas cap light every other fill up
-clamped the wires and connector for the upstream o2 sensor, right side between the bell housing and block at clutch change 10k miles ago, had plastic cover, no apparent damage, no codes but a lean out code on bank 2 popped up 1000 miles down the road.
-no engine oil or coolant leaks
-manual shift
-regular and on time maintence
-unknown history for first 210k miles but seemed pretty well taken care of. 100% stock when i got I bought it
-fully stock other than CB radio, t bar lift, camper top, steelies and 33s.

I would look at fuel pressure see if holds pressure after pump shuts off. Maybe finding out what is causing gas leak and gas cap light.

Try googling it.You will connect to the valve at the end of fuel rail with a pressure gauge See if it will maintain pressure in the line.

get in the truck and cycle the key, turn it to on but don't start it.. wait a few seconds, then turn to start! does it start right away

It seems to be holding pressure properly and consistently in that respect.
The issue occurs when the tank is being filled, if i let it fill until the pump makes a click, it will spill a couple quarts out onto the ground from the area near the fuel pump. After that, gas can sometimes be smelled faintly at stop lights and when I get out

Normally, yes will chirp to life with even the quickest touch of the starter (even without priming the pump), but still struggles in the cold regardless of if I do that or not.

Compression is slightly on the low side low, but even across all cyl and still within spec on all cyl.
