Default 94 4.0 SOHC idle misfire/heavy fuel/issue revving past idle

1994 ranger
4.0L, 4 speed, extended cab 2wd
104k miles, pretty decent condition

Starts fine. Sometimes engine stutters, misfires, idles fairly roughly, engine won't rev past idle when accelerator is pressed (I've almost been t-boned because of this).

worse with AC on, headlights, etc, increasing idle engine load.

high rpm is fine, once the motor is able to rev it acts fine.

gas mileage is average -2ish mpg where I should be. even with cats exhaust had heavy raw gas smell to an extent, more than acceptable

no cats
maf good
o2 good
battery good
alt-unsure but think it's ok (good grounds)
spark plugs and wires should be ok, newer

throttle body issue (i had it cleaned not too long ago)?
intake manifold gasket?
vacuum leak?
spark plug gap?

too much fuel? (runs the same with maf unplugged, seemingly as it does "working properly")

Any recommendations would help. Thank you.

Have you tested the IAC valve ? Looks like this lil bugger here..

If it runs the same with the MAF unplugged then that sounds like a bad MAF to me? Kinda guessing here...
