Update...

I finally threw my hands up and put the ranger in the shop. He had it for a week and said all he could think of was the computer (ECM). He told me that I had replaced everything pertaining to the possible cause up to the computer. He pulled the plugs as well, which were fine. He said there is not a way to test an interment computer (ECM) issue.
I'd hate to dump money on an ECM with "fingers crossed" if you know what I mean.
I had replaced the ECM in 2012 with a reman. I have not found an OEM that is new and due to the age of the truck, probably won't.

Well, I replaced my upper intake manifold gaskets (and TB gasket) today, and it didn't fix it for me.

I have the whole gasket kit but I didn't have time to go past the fuel rail...that's a big can of worms.

I'm probably going to try the fuel pressure regulator next.

UPDATE: I JUST TRACKED DOWN MY VAC LEAK!

I had been spraying everywhere I could to find this with no luck, but I didn't try spraying UP into the hole where the EGR tube goes into the manifold. Sure enough, it surges when I spray in there. I think/hope it's just the little donut gasket that needs replacing.

I'm hoping I can pull the tube and replace the gaskets without taking the whol manifold off again.

LATER: Safely removed the EGR and tube thanks to PB Blaster. New gaskets will be going in tomorrow. Did you know there's a tab on the dip-stick near the bottom that you can pry up on?

NEXT DAY: Got it installed with the new gasket this morning, fired it up and spray-tested, and there was no idle surge... for now it's sealed. We'll see how it holds up.

@Txoutback,

What were your symptoms? Was it isolated to anything particular ... meaning after the truck had been driven; or only at cold crank or both?

Did you attempt to use a KOKO reader or just going by the obvious that the CEL isnt lit? You might have a soft code in there. If not, that "should" eliminate a sensor issue and now your going to be looking for a mechanical or physical issue.

UPDATE - I super flushed my cooling system as well as replaced my heater core due to it not working. Also my temp gauge would never rise past the "\" of the "C\" of my dash (hopefully you guys understand my needle & gauge reference). Side note, I have replaced both the thermostat as well as the thermostat sensor on at least 3-4 different occasions in the course of 1.5 years thinking that I had a faulty part. After replacing the heater core, and driving the truck, I let it sit for about 15-30 minutes and then fire her back up. The surge, stumble, rough idle, etc has been cut in half as of yesterday (one day of driving). Let me also say that I did find that the heater hose traveling to the motor (right side hose looking at the motor) had pressure still built up after draining the system (depressurizing). I do not know if that hose is suppose to be "full" even if the coolant is drained out of the system because I am not a mechanic but any words of advice is appreciated.

@Understm8ed - I did not attempt to use the KOKO reader and yes, the CEL has never lit up due to this issue. I have not heard the term "soft code" though. Can you explain what that is?

A soft code is when the CEL comes on for however long or brief and goes out and may or may not randomly persist vs a hard code issue keeping the CEL on constantly.

It was explained to me that way long ago by a group of fellow Honda tech guys and its stuck in my head like that.

After driving the truck for a couple of days, and letting it sit for about 20-30 minutes and starting the truck back up, there is no change. The truck still runs extremely rough when you start it back up from sitting (after being driven). I have seen and read a number of posts on this forum with very similar symptoms that I am experiencing with the same motor yet no one has been able to pin point a solution. Am I missing something here?

Idle air flow controller. Controls how much air comes in at idle. What motor do you have?

Edit: 4.0 OHV. Should be on the drivers side of the plastic upper intake manifold.

The problem still has not been fixed. I have found that if I put the key in the ignition and turn to the accessory position and wait for the chimes/dinging to stop (10-15 seconds) and then start it seems to run better (for the most part) as opposed to jumping right in and firing it right up. I would think the issue has to do with fuel delivery. Thoughts?

So far I have replaced:
- Plugs - motorcraft
- wires - NGK
- ignition coil - motorcraft
- PCV valve - motorcraft
- fuel pressure regulator - motorcraft
- fuel filter - motorcraft

I have cleaned:
- MAF & pigtail connector and coated w/ die-electric grease
- throttle body
- IAC & pigtail connector and coated w/ die-electric grease

These parts either needed to be replaced due to necessity or routine maintenance.

Any helpful thought are appreciated.

Fuel pump or fuel pressure regulator would be my next guesses. Both are easy to check. Start by checking fuel pressure at the rail.

Did you replace both temp sensors? One is a sender for the gauge but there is a second that sends temp info to the ecu. I've had to replace both on mine. It still sounds like a vac leak to me though
