Could be camshaft synchro. Tells your pcm when your at tdc, won't throw a code as long as the sensor is still operating.

Any idea age of the fuel filter?
Fuel pump could be weak.

Wait until it's dark start the motor turn off all the lights and see if you see a spark jumping to ground around the plug wires, plugs or coil.

I have a similar issue, been chasing it for years and finally gave up and just said it has run this way for this long, it will keep going longer haha. I have changed my heads (cracked as well) oem ford headgaskets and new intake gaskets, plunumb gaskets. Replaced 2 sets of plug wires 3 sets of plugs, crank sensor, cam synchronizer, ECU, vacuumed lines (check for bad vacuum lines to the vacuum canister in the passenger fender, mine were starting to rot when I did them) also lines to fuel vapor canister next to the battery, new fuel injectors, fuel filter, checked fuel pressure, new air filter, tried running with no CAT to see if it was plugged, replaced timing chain and sprockets, I think the only thing I haven't done is replace the rockers and pushrods.

I'll be following your thread to see if you come up with anything, but check the basics first, filters plugs and wires, fuel pressure, things that are cheap and good to do anyways
