I havent on a 4.0 but to replace the release bearing you have to pull the transmission. You can get just the bearing but if the slave cylinder is in the bell housing I would suggest getting the slave cylinder because it comes with a new bearing. If the slave is in the bell housing there should be 4 bolts to get the bell housing off and the slave cylinder just pops out. If its external just get the bearing and no slave cylinder. But either way the transmission has to come out. Sorry but no way around that.

Thanks man I knew i was going to have to pull the trans but what all should I replace? I was thinking I would change the cluch, flywheel and slave. Im not sure if i need to or not the clutch doesnt slip the bearing is just loud and its hard to get it to go into gear. Any suggestions?

If you gotta pull the tranny my thought would be just to go ahead with it all. If its hard to get into gear its probably that release bearing and possibly worn clutch. But yes clutch kit (includes pressure plate, clutch plate pilot bearing and release bearing), slave cylinder and flywheel.

And save future problems. Oh one other thing. If your goin to have the flywheel off go ahead and replace the rear main seal too. That will definatly save you some headache later.
