your truck has a OBD1 system so it's different then the OBD2 system 111 code means everything is ok as far as the computer sees

check for vacuum leaks first Take a can of brake cleaner while the truck is running and spray a little on the vac lines If the engine revs up that line is leaking

If all the lines are ok make sure you are getting fuel to the engine and at the proper pressure make sure the spark plugs and good and getting power and make sure you are getting enough air into the motor

here is a diagram for the vac lines

X2 on checking for vacuum leaks also replace the fuel filter and check the fuel pressure.

While checking for vacuum leaks spray around the intake. See if there's a leak in the gasket.

Your 94 Ranger uses the EEC-IV engine management system, not OBD-II which came out in 96. With EEC-IV there are three types of codes, KOEO, KOER and CM. did you check all three groups?

Just because it came up with a 111 for KOEO doesn't mean there isn't a valid KOER code. The wandering idle suggests (but is not guarantee of) a vacuum leak. 4.0 Fords of your vintage are infamous for lower intake manifold gasket leaks, in fact, every one I've ever retorqued had at least one bolt that was finger tight. Knowing this, you should examine for a leak and as a PM action, retorque the lower intake to head gasket bolts.

*Did you do anything to this vehicle right before this issue surfaced?

It's a 94 4.0, it is a DIS, there is no cap/rotor.

You didn't mention air filter. Did you replace that as well? And just because you've replaced a part with a new one, doesn't mean it's good. Bad parts happen. I can't count high enough to register the times I've replaced parts with new just to be bad... Check 'em. Plug wires, plugs, coil, iac, maf, etc.... Don't assume it's good cuz it's new.

One other thing... Have you considered electrical faults? It's the number one overlooked diagnosis in all automotive repair worldwide. Check your grounds, check your harness plug, look for shorts, etc. I remember one vehicle at a ford dealership, a noob tech fresh out of UTI replaced over $3k in parts including the PCM, ECM, coils, etc, and it kept producing the same problem every time. Intermittent sputtering, erratic idle, loss of pwer. I finally took a look at it so we wouldn't lose anymore money, and found the culprit: a corroded ground connection. Problem: solved. Car ran fine after fixing that one ground.

Moral of the story: DON'T OVERLOOK ANYTHING!!!

Have you an answer for the question posed in post #9? Yes, it does matter.

Yes, 95 was a mid production year migration for some Fords and as such, some have EEC-IV and since Ford migrated ahead of the mandate, some have OBD-II. Really. People looking for parts sometimes make the *discovery*, lol.
