The hoses could be worn out or leaking causing the code to set. Ive seen the hoses dryrotten and cracking so i usually replace the rubber hoses with the dpfe sensor.

YES, as they can dry rot and if they leak it will give a code.

so do these hoses need to be the same diameters or can I just get close to it?

close to it is fine. just take the sensor with you if possible and get hoses with a snug fit. shouldnt need clamps if you get the right hoses.
