Re: reader codes
P0300 is random misfire.
P0304 is cylinder 4 misfire
P0316 is misfire on startup
I would focus on cylinder #4 to start with. Get it compression tested and swap the injector with another one, or replace the injector.

Thank you. I knew what the codes were and I will be testing the coil for cyl #4 today as well. O2 sensors (both) are a potential concern as well. What's the best way to test the cylinder compression?
There also TSB's from Ford regarding these same codes involving cyl #4 and the cylinder head and engine exhaust valve seat. I hope it is not that though.

Re: reader codes
O2 sensors don't cause single cylinder misfires. Where are you even getting that concern from? A compression tester screws into the spark plug hole, pressurizes the cylinder, and you can see if it holds or loses pressure.

were you getting all these codes before you started changing things? or did it happen after? have you gone back to check all the connections to make sure are are seated and pushed in tight?

Because the other codes (random) misfires can also be caused by improper oxygen mixture. If a o2 sensor is faulty it potentially could be an emissions issue regarding passing smog tests. Not to mention my code reader flashed the o2s, cat and htr. I realize the #4 cylinder misfire may in fact be a separate issue that may involves the electronic distributorless ignition. These are all potential power loss, mpg and emissions issues, specially in Calif.

Yes, they were there prior to changing anything. That is what prompted the work my son and I have done. We will re verify all connections though it sounds and drives beautifully. Only a small shake when idling after warm up. 699 rpm per the freeze frame info.

Re: reader codes
did you reset the cel afterwards to erase old codes from memory?

We did, but only after printing out the report to save them. We did the oil change and fuel filter. Four days later (yesterday) my son said the light came on again. It gave all the same codes with the addition of the #4 cylinder misfire. We changed the wires and plugs and the tranny fluid / filter. I guess the next step is verifying the cylinder pressure. If that is ok then I will need to check the coil to see if that is working.g properly.

Changed out all four O2 sensors and coil. Runs, drives and sounds great. O2, htr, cat and Eva are flashing. No codes. Weird.

On my innova 3150b scanner those are flashing on and off. I need to check on the meaning though.

You should get your hands on a scanner that can give you LIVE DATA, that way we can help a lot more.
I have INNOVA 3160B and have never had a Flashing reading, which leads me to believe that there's an electrical gremlins in you truck which may be nothing more than bad grounds or poor voltage output from ether the battery and or alternator.
Seeing how you still have the original issues ,tells me that the underlining problem hasn't be fixed.
Computerized cars and trucks require a clean and constant power source or they can and will act very strange.
With this in mind, I would make sure that the alternators output is good and that the battery is also tested, clean all the grounds that you can find and once your sure that’s taken care of you can move to the next phase of test.

Actually the original issues are not present anymore. Those were random misfire codes and cyl #4 misfire. You bring up a valid point though. The battery is new. I have contacted innova regarding these issues so we shall see.
