Rough Idle, Flashing Engine Light, Strong Exhaust
The truck starts fine, but the exhaust has a strong smell. After driving a few minutesand stopping at a light, the engine light begins to flash and the engine idles very rough. It has a loss of power and is hard to start off from a stop. If I keep the RPMs above 2000, the engine light stops flashing and the engine levels out. I had the codes checked at Autozone and it came back cycl 4 misfire. I have changed plugs, wires, coil pack, fuel filter, air filter, #4 injector, cleaned the MAF sensor & IAC valve. I have checked for vacuum leaks and found nothing. What next?

Check you exhaust. Does it have a sweet smell to it? If so change your oil and see if there is any coolant in it. Sounds like you might have a blown head gasket.

A bad cat does not cause a cylinder 4 misfire.
You've replaced all the stuff specific to cylinder 4 that you can replace. Which leads to more difficult things. Bad head gasket leaking into that cylinder? Blown piston rings in that cylinder? Things like that.

I bet you have an exhaust valve issue .....I have seen it a few times.....I think there was/is a TSB for it.
