I would not have drilled holes in the exhaust system but it's done. I would suspect the MAF as suggested. You can read the MAF with Torque or other scanners. You can also disconnect the MAF as Hammer suggested and if the truck runs better I would replace the MAF. With a properly functioning maf the truck should die shortly after disconnecting it.
Have you checked your cam syncro lately?
FYI : your gas pedal is really an air pedal. When you press it down it calls for air. If the MAF is fubared you will get the result you're getting.
