Reading this and 100,000,000 thoughts are running through my head. It could be a lot of different things. I can't even remember what all you have checked and I don't really want to go back through all of your threads to figure it out; it would be easiest to keep everything in one thread.

Are the only things done so far are cleaning the IAC and TB?

First place I would look is the computer for any clues. Eg, if you get a code 41 you most likely have a vacuum leak because the o2 sensor is sensing too much oxygen in the exhaust. It's too broad of a problem to really point at anything in specific, I can sit here and give you ideas of what it could be all day long and be wrong 99.99% of the time. First need to make sure ignition components are up to snuff (plugs, wires, cap, rotor), test resistance on your ignition coil (when it's acting up), check for vacuum leaks w/ carb cleaning and listen to idle, check computer for codes, check running fuel pressure, look at vitals and make sure you have you bases covered and that will help narrow down where the problem is.
