My truck had a lot of trouble starting and idling when it was cold. I would have to hold the gas pedal half way down just to keep it running until it warmed up. It was a bad ECT sensor. The ECT sensor tells the computer the engine temp and adjusts the IAC valve depending on the temp of the engine.
